Shared Decision Making Team has created an online survey to find out how we’re
doing! Your opinion is so important to us. Please take a few minutes to
take the survey found here. The short survey has 25 multiple choice questions.
There are several opportunities to comment on different sections as well. We appreciate your
participation, and look forward to your valuable input and feedback.

the results of this survey and use them to guide the Shared Decision Making
